Abstract: 

Background & Aim: Pseudomonas aeruginosa strains that were resistance to majority of commonly used antibiotics were caused problem in treatment of these infections. Imipenem is the excessive potential antibiotic for elimination of antibiotic resistance isolates of these bacteria. Aim of this study was, identification of imipenem effect on TEM beta-lactamase gene expression in resistant to antibiotic, Pseudomonas aeruginosa strains, from burn wound samples, by Real-time PCR. Methods: In descriptive cross-sectional study, 100 burn wound samples were collected from Tehran hospitals. Pseudomonas aeruginosa isolates, were found from samples. After DNA extraction by PCR reaction, were isolated TEM gene from samples contained this bacteria, and were defined imipenem MIC measure, for them. Then, were compared, TEM beta-lactamase gene expression, among imipenem-treated and untreated Pseudomonas aeruginosa strains, by Real-time PCR Results: From 100 wound samples, 60 isolates (60%) were infected by Pseudomonas aeruginosa and in 34 samples (56. 6%) of Pseudomonas aeruginosa isolated, were presence TEM beta-lactamase gene. Expression of TEM gene, 78. 68% decreased in imipenem-treated Pseudomonas aeruginosa strains, compare by untreated strains. Conclusion: According of this study finding, from imipenem by anti-beta-lactamase action that was interested on reduction of TEM gene expression, could be used in control of antibiotic resistance Pseudomonas aeruginosa strains.

Abstract: 

TEM beta lactamase gene is one of the important plasmid genes in Enterobacteriaceae which is the cause of over 90% of Escherichia coli isolates resistance to beta lactam antibiotics. The aim of this study was to detect the prevalence of antibiotic resistance and TEM gene. 266 clinical isolates of E. coli were collected from laboratories in Bonab County. Phenotypic screening and confirmation tests for extended spectrum beta lactamases (ESBLs) were carried out using disk diffusion (Kirby Bauer) method. All of the ESBL producing isolates were tested by PCR using specific primers. Our results showed that, the maximum resistance was seen for ampicillin (67. 3 %) and the maximum sensitivity was seen for imipenem (92. 5%). In this study 45 % isolates were multidrug resistance, which showed at least resistance for three antibiotics. Out of 154 isolates, 58 (37. 7%) cases were ESBL producers which 65. 51% of isolates contained TEM gene. This study showed that, TEM gene encodes over 50% of ESBLs in E. coli. Therefore, we recommend detection of this gene as a routine bacteriologic procedure in management of the nosocomial infections caused by enteric bacteria.

Abstract: 

Background and Objectives: Emergence of antibiotic resistance is a main problem in microbial infection control. Production of extended spectrum beta-lactamases (ESBLs) can cause antibiotic resistance in bacteria. This study was conducted to evaluate antibiotic sensitivity profiles and the presence of blaTEM gene in Escherichia Coli and Klebseilla. Pneumoniae isolates collected from different clinical specimens.Materials and Methods: In this prospective study (2008-2009) clinical specimens including tracheal aspirates, urine, blood, bronchial secretions, sputum, peritoneal fluid, wound and throat swabs were collected from educational hospitals of Tabriz University of Medical Sciences. We performed antibiotic sensitivity tests (Kirby-Bauer Method) on E.coli and K. Pneumoniae isolated from specimens. Antibiotic discs that were used in this study werecefotaxime, ceftazidime, ceftriaxone, azteronam, amikacin, piperacillin/tazobactam, cefuroxim, cefepim and imipenem. Combind disk test was also used by ceftazidime, ceftazidime/clavolanic acid, cefotaxime and cefotaxime/clavolanic acid. E. coli ATCC 35218 and E.coli ATCC 25922 strains were used as TEM-positive and negative controls respectively. BlaTEM genes were detected by using PCR technique.Results: From total number Of 41 E. coli isolates 36 (87.8%) contained blaTEM gene. All of the Ecoli isolates were sensitive to imipenem, while, 36 of them (87.8%) were resistant to azteronam and 40 of them (97.5%) were ESBL producers. From total number of 47 K. Pneumoniae isolates, all were sensitive to imipenem, while 43 of them (97.8%) were sensitive to cefotaxime, 41 isolates (87.2%) showed resistance to azteroname and 46 isolates (97.8%) were ESBL producers of which 35 isolates (74.4%) contained blaTEM genes.Conclusion: High rate of resistance in E. coli (80.4%) and K. Pneumoniae (91.4%) isolates .Undelines the importance of accurate sensitivity tests.
